Patient Care Coordinator

Description:

This full time position is committed to removing the client’s barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems and promoting client health; responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and other mental health providers; provides care management/coordination for patients of all ages, helping them address complex emotional and physical needs in the primary care setting, under minimal supervision; serves in an expanded health care role to collaborate with primary care providers, specialists, members of the health care team and patients/families to ensure the delivery of quality, efficient and cost effective health care services.  Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient’s health status promoting quality and efficiency in the delivery of health care.

Qualifications:

Associate’s degree in Nursing minimum; licensed registered nurse (RN) in the state of MI.; valid MI. driver’s license (in compliance with HIC insurance carrier) and dependable transportation required.  CPR certified or the ability to become CPR certified within 6 months of hire; experience in behavioral health care management; strong understanding of cultural competency with Native American families.

Wage: Negotiable
